Appointments are required. Use the form below to request a food pantry visit. A member of the Revive My People team will contact you to schedule your appointment.

Households may visit once per month.

At your appointment, you’ll receive a selection of food and other essentials based on your household size and needs. We cannot guarantee to provide everything marked down on the sign-up form that you have requested.
What to Bring

A form of ID (if you have one)

Proof of address (if available)

Basic household information: how many, age range, etc.

Any information about dietary needs or restrictions in your household
If you don’t have all of these items, please still complete the form to request a food pantry visit. We will work with you.
